INJURY DOCUMENTATION

The details of your injuries can be very important. This can affect your diagnosis and recovery, but also establish how your injuries are related to a recent traumatic event. When you come for your initial visit, please help us by bringing all records from your primary doctor, chiropractor, and imaging center. If you can, also bring the police report or incident report that was filed or the documentation of your injury by the medics or the ER showing the angles and speed of your car accident. Finally, if there are any pictures of the damage on your body or your vehicle, that can help us understand and document the mechanics of your injury better. All of this is geared to providing an accurate diagnosis and treatment plan.
